Card Replacement Request
Requesting for Card Replacement
This allows the company users to request a replacement card in case of a lost, stolen, or damaged Damstra card. The company users will be required to complete an online form and make a non-refundable payment before submitting the request to Damstra Technology.
- Select the Employees tab, then click Replacement Card Request
- Select the person’s name you are requesting for card replacement from the dropdown list
- You can upload a new Photo for this employee (this is optional)
- Click the green Request Replacement button to proceed
- A pop-up window will appear for confirmation
- Select where you want the card to be delivered
- Select which payment mode you want to then provide the details and click Submit Payment for CC. For PO, provide the details required then hit I Agree, Save Purchase Order.
